Offering an $8K sign-on bonus, this RN role involves providing intermittent home nursing care based on physician-approved plans.
Supervises Home Care Aides and reports to a Clinical Manager.
Requires a nursing degree, current Connecticut RN license, and at least 1 year of medical/surgical experience; community health experience preferred.
Must have good physical mobility, ability to operate a vehicle, and tolerate working in patients’ homes with exposure to communicable diseases.
Responsibilities include conducting assessments, developing care plans, evaluating patient progress, documenting services per regulations, and coordinating care with physicians.
Ensures quality, safety, and compliance standards, manages caseload efficiently, and supervises Home Care Aides.
Maintains professional skills, adheres to policies, and participates in ongoing education.
Additional duties include community outreach, administrative tasks, and providing backup coverage as needed.
