Job Description
Seeking an RN with a $8K sign-on bonus to provide intermittent home nursing services under physician-approved care plans.
Reports to Clinical Manager and supervises Home Care Aides.
Requires a nursing degree (NLN-approved, associate or baccalaureate), current Connecticut RN license, and at least 1 year of recent medical/surgical experience; community health experience preferred.
Must possess strong clinical skills, knowledge of safety protocols, and proficiency in documentation, patient assessment, and care planning.
Works in patients’ homes and office settings, handling physical demands such as lifting and operating a vehicle, while ensuring safety and infection control.
Key responsibilities include patient assessments, developing and updating care plans, evaluating patient progress, documenting services, supervising aides, and coordinating with healthcare providers.
Maintains professional development, adheres to compliance policies, and may perform additional agency duties as assigned.
Must have transportation, valid insurance, and be willing to serve various geographic areas.
