Schedule:
Mon-Fri 8a-3p or 3p-11p with alternating weekends.

Evaluate medication orders, maintain patient databases, collaborate with healthcare teams, document communications, and ensure compliance with regulations.

Qualifications:
Bachelor’s Degree in Pharmacy (Pharm D preferred), one year of long-term care experience, and relevant licenses/certifications.
