Job Description

St Croix Hospice is seeking a Registered Nurse Case Manager for their Newburgh, IN branch. The RN will plan, organize, and direct hospice care, focusing on community health education. Key responsibilities include:

• Conducting comprehensive assessments of patients and families to determine hospice needs.
• Developing and revising care plans based on patient evaluations.
• Administering medications and treatments as prescribed.
• Communicating with physicians and coordinating care with community health professionals.
• Supervising hospice aides and participating in on-call duties.
