RN Home Hospice Case Manager – Full Time

Job Description

We are seeking a full-time RN Home Hospice Case Manager in Pewaukee, working Monday to Friday from 8:00 AM to 4:30 PM, with no weekends and minimal on-call duties. A $10,000 sign-on bonus is available with a two-year commitment.

The RN Case Manager will coordinate care for patients in a home environment, collaborating with various healthcare professionals to ensure high-quality, cost-effective care. Ideal candidates should have strong communication, critical thinking, and teaching skills, along with experience in pre-acute and post-acute care.

Requirements include an Associate’s Degree in Nursing (Bachelor’s preferred), a valid RN license in Wisconsin, and current BLS certification. Clinical experience and familiarity with EPIC software are preferred.
