RN / Registered Nurse Case Manager – Hospice

We are seeking a dedicated RN Case Manager in Reading, PA, to provide essential one-on-one care in patients’ homes while supporting their families. This role offers flexibility, autonomy, and the chance to enhance your nursing skills.

Responsibilities include:

• Planning and delivering safe patient care
• Collaborating with the Interdisciplinary Team

Schedule: Monday-Friday, 8 AM-4:30 PM, with on-call duties for holidays.
