Physical Therapist

Job Description

This position involves providing innovative, patient-centered physical therapy services in a home health setting. Key responsibilities include:

• Assessing patients’ therapeutic and functional status and developing care plans.
• Directing treatment, instructing patients and caregivers, and reporting patient progress to physicians.
• Initiating therapy programs, training in prosthetic use, and identifying additional home health needs.
• Submitting clinical documentation and participating in discharge planning.
• Supervising physical therapy assistants and providing in-service education.

Qualifications include a degree in physical therapy, state licensure, and two years of relevant experience. The role is full-time, on the road, with a pay range of $55.00 – $105.00 per hour and various benefits.
