RN Case Manager Hospice – Up to a $25,000 sign-on bonus

Job Description

Position Summary:
This role offers a $25,000 signing bonus for a Hospice Nurse who will work within an interdisciplinary team to provide skilled nursing care, assessment, and planning to enhance patient and family comfort. Responsibilities include identifying needs, supervising Home Health Aides and LPNs, and ensuring care aligns with physician orders and hospice policies.

Education, License & Cert:
Must be a Registered Professional Nurse licensed in Pennsylvania, with knowledge typically associated with an Associate’s Degree.

Experience:
Preferred one year of nursing experience in acute care or hospice settings, with supervisory skills desired.

Essential Functions:
– Manage a caseload of 1-15 patients/families.
– Communicate with physicians and hospice staff.
– Maintain patient records and care plans.
– Supervise hospice aides and LPNs.
– Attend team meetings for professional support.

Other Duties:
– Travel and on-call duties required.
