We are seeking a Licensed Physical Therapist (PT) for a new Care Coordination Program, offering up to a $3,000 sign-on bonus. This role allows you to provide physical therapy and wellness services to seniors in various living settings while engaging in care coordination tasks. Responsibilities include conducting assessments, treatment planning, and wellness visits, as well as collaborating with a multidisciplinary team.
