Job Description

A nonprofit community health care organization seeks an RN Case Manager to provide nursing care and case management for patients and families in hospice settings. The role involves conducting assessments, creating and updating care plans, and ensuring patient satisfaction. The RNCM collaborates with an interdisciplinary team and participates in patient visits, education, and care planning.

Hours: Monday-Friday 8am-5pm, with weekend and holiday rotations.
Qualifications: ADN or BSN, current RN license, preferred one year of nursing experience, and valid driver’s license.
Bonus: $6000 eligibility.
