Adara Home Health is seeking an RN Case Manager to provide direct care and oversee home health services for clients in the New Prague, Belle Plaine, and Jordan areas. This full-time position offers a flexible Monday-Friday schedule with on-call duties every fifth weekend and one weeknight every other week. Responsibilities include developing care plans, conducting assessments, advocating for clients, and providing clinical oversight.
