On-Site Care Coordinator (RN – Florence County, SC Territory)

Job Description

Job Summary:

This work-from-home position requires travel within Florence, Dillon, and Marion, SC Counties. The On-site Care Coordinator, a Registered Nurse, provides care coordination and education to Medicaid members in the Medically Complex Children’s Waiver program.

Responsibilities:

• Conduct comprehensive assessments and determine care requirements.
• Review LOC forms and medical records.
• Educate parents/guardians on child care.
• Develop and submit care plans for approval.
• Coordinate services and maintain communication with families and providers.
• Identify complaints and develop disaster plans.

Qualifications:

• Minimum of three years of clinical pediatric experience.
• Bilingual (Spanish) preferred.
• Valid driver’s license and insurance required.
• Criminal background check and initial TB test required.

Education:

• Bachelor’s degree in a health-related field preferred.
• Current RN licensure and BLS certification required.

Skills:

• Strong organizational and communication skills.
• Proficient in MS Office applications.

Physical Demands:

• Office environment with potential for repetitive motion and lifting up to 30 pounds.
