Primary Care Nurse Practitioner / Physician Assistant

Job Description

We are looking for a compassionate Primary Care Provider to join our team, focusing on delivering comprehensive care to residents in long-term care facilities. This role involves:

• Providing healthcare services and coordinating patient care
• Assessing, diagnosing, and treating residents’ physical and mental health
• Conducting patient assessments and ordering diagnostic tests
• Developing treatment plans and prescribing medications
• Collaborating with an interdisciplinary team
• Creating a safe environment for elderly residents

Requirements include a Nurse Practitioner or Physician Assistant License, DEA License, and 6 months of long-term care experience. Benefits include health, dental, vision, and paid time off, with a salary range of $150,000 – $200,000.
