Clinical Service Coordinator – Intensive Home-Based Therapeutic Care

Job Description

General Description: Provide mental health treatment, psychotherapy, case management, and treatment planning for youth and families receiving intensive community services.

Essential Job Functions:

• Conduct diagnostic assessments and deliver individual, family, and group treatment.
• Provide outreach treatment in clients’ homes and subcontractor facilities.
• Maintain client records and complete necessary paperwork promptly.
• Coordinate treatment with collateral agents and subcontractors.
• Offer consultation to community agencies and subcontractors.
• Implement Wraparound care planning for youth and families.

Qualifications:

• Master’s degree in social work, psychology, or counseling; LCSW, LICSW, or LMHC preferred.
• Required licensure within 6 months of eligibility.
• Experience in mental health/substance abuse counseling.
• Knowledge of community resources.

Skills:

• Excellent oral and presentation skills.
• Leadership and delegation abilities.
• Proficient in computer use.
• Ability to work in an interdisciplinary team.
