We are seeking a motivated *MDS Clinical Care Coordinator* to join our Nursing & Rehabilitation Centers. This full-time position offers a competitive wage of $40.45 – $46.95 per hour, with additional benefits including generous PTO, health insurance, and a 401K match.

*Key Responsibilities:*

  • Assess residents’ physical and mental functions.
  • Document data accurately on minimum data set forms.
  • Make referrals to healthcare professionals.
  • Supervise resident care and participate in interdisciplinary meetings.
  • Oversee clinical coordination and infection prevention programs.

  • Registered Nurse (RN) or Licensed Practical Nurse (LPN).
