We are seeking a Per Request Needed (PRN) Occupational Therapist for our Canon City office in South Western Colorado, serving patients in Canon City and Pueblo. Our team is dedicated to providing high-quality, compassionate home health and hospice care.

Responsibilities:

• Evaluate and provide services for patients with various impairments and disorders.
• Complete visit notes accurately and timely.
• Educate patients and caregivers.
• Assist with discharge planning and consult with other professionals as needed.

Pay: $43.00 – $46.00 per hour. Must have an Occupational Therapy License and be able to commute to Canon City, CO.
