We are seeking a Licensed Speech-Language Pathologist (SLP) to join our innovative care coordination program, focusing on providing therapy services to seniors in various living settings. This role allows you to practice at the top of your license while engaging in care coordination tasks, offering flexible scheduling and excellent benefits.

Responsibilities include:

• Delivering speech-language pathology and wellness services to older adults.
• Conducting assessments and therapeutic interventions as part of a multidisciplinary team.
• Supporting care coordination through wellness visits and communication with providers.
• Maintaining professional standards and contributing to program development.

Qualifications:

• Completion of an accredited SLP program (New Grads Welcome).
• Active ASHA certification and state licensure.
• Strong communication skills and a results-oriented mindset.
