Requirements:

• Degree in physical therapy from an accredited program
• State licensure as a physical therapist
• Minimum of two years of experience, preferably in home health or hospice
• Strong communication and organizational skills
• Current CPR certification
• Valid driver’s license and insured vehicle

Responsibilities include:

• Conducting evaluations and developing care plans
• Educating patients on prosthetic devices
• Documenting clinical progress using OASIS
• Participating in care planning and interdisciplinary teams
• Supervising physical therapy assistants
