Job Description

We are seeking a full-time MDS Coordinator for a 48-bed Skilled Nursing Facility, offering a salary of $80,000 – $85,000 and a $6,000 sign-on bonus. The role involves managing Minimum Data Set (MDS) assessments, ensuring compliance, and collaborating with an interdisciplinary team to maintain resident care plans. Candidates must be a licensed RN in New York with experience in long-term care and MDS processes. Strong communication and documentation skills are essential.
