Location: Valparaiso, IN
Schedule: Every weekend or every other weekend; 8 or 12-hour shifts
Summary: The RN Weekend Clinical Liaison serves as a key link between hospitals and the agency, focusing on care collaboration, discharge planning, and effective communication. Responsibilities include building relationships with healthcare professionals, educating patients and families about services, coordinating with intake, and providing nursing care to patients needing GIP level care in hospitals.
Requirements:
- Unrestricted RN license in Indiana
- Diploma from an accredited nursing school
- Excellent communication and organizational skills
- Minimum 2 years of experience in medical-surgical, critical care, or oncology nursing
- Hospice experience preferred
- Current CPR certification
- Valid Indiana drivers license and automobile insurance
- Current TB status
Mileage reimbursed at IRS rate.
