Location: Skilled Nursing Facilities, WI Rapids/Stevens Point areas

Full-Time: Monday – Friday, No night, weekend, or holiday coverage

Position Overview: Seeking an experienced Advanced Practice Provider (NP/PA) in internal medicine to deliver onsite medical care in skilled nursing facilities, focusing on acute and chronic care for adult and geriatric patients.

Key Responsibilities:

  • Provide primary care for adult and geriatric patients.
  • Develop personalized treatment plans with the care team.
  • Establish diagnoses, prescribe medications, and perform procedures.
  • Document patient assessments and care plans accurately.
  • Coordinate care with specialty providers.
  • Communicate effectively with patients, families, and staff.

    • Qualifications:

  • Advanced Practice Provider license (NP or PA).
  • Valid Wisconsin APNP or PA-C License and DEA.
  • AHA BLS certified.
  • Minimum 1 year of experience preferred.
  • Strong clinical and interpersonal skills.
