Adult Case Manager

Job Description

A full-time Adult Case Manager is needed to provide care for clients diagnosed with mental illness. Responsibilities include maintaining a caseload, coordinating services, updating documentation, collaborating with treatment teams, monitoring client progress, and providing crisis intervention as necessary.

Qualifications:

• Bachelor’s Degree in Social Work, Psychology, or related field
• 1 year of experience with mental illness or substance use disorder
• Must qualify as a Mental Health Practitioner in MN
• Preferred: Licensed Social Worker in MN or Provisional License
• Valid driver’s license and auto insurance

Benefits: Starting pay at $23.65/hour, medical, dental, vision insurance, PTO, 401(k) with match, and more. Schedule is Monday to Friday with no weekends.
