We are seeking a *Home Care Physical Therapist* to join our *Home Care Team*. This role involves providing physical therapy services to patients in their homes, following a physician-approved Plan of Care. Responsibilities include initial evaluations, ongoing planning, and implementation of care plans.

Requirements:

• 2+ years of Physical Therapy experience
• Maine PT License and Basic Life Support Certification
• Exceptional customer service and computer skills
• Valid driver’s license

Job Type: Full-time, Monday to Friday, with on-call and weekend availability. Work location is in-person in York Village, ME.
