Locations: Autumn View Health Care Facility, Hamburg, NY; Harris Hill Nursing Facility, Williamsville, NY

Responsibilities:
Provide physical therapy and supervise assistants/aides.
Develop and apply therapy modalities, therapeutic exercises, and functional training.
Document progress and assist in care planning.
Requirements:
New York State PT licensure and CPR certification.
Ability to lift 50-100 lbs and collaborate with healthcare teams.
Strong communication skills and empathy for residents.
