Position: Licensed Clinic Director – Care Coordination Program

Overview: Seeking a licensed therapist to lead a care coordination program for seniors, enhancing their quality of life across various care levels. This role offers a $2,500 sign-on bonus, flexible scheduling, and opportunities for career growth.

Responsibilities:

• Oversee patient care in senior living facilities.
• Provide therapy services and operational support to rehabilitation teams.
• Contribute to treatment planning and quality improvement initiatives.

Qualifications:

• Degree in OT, PT, COTA, PTA, or SLP.
• Active state licensure and ASHA certification for SLPs.
• Strong communication and problem-solving skills.
• Management experience preferred.

Compensation: $45.00 – $55.00 per hour + bonus.
