Responsibilities:
Conduct patient assessments and coordinate care with the interdisciplinary team
Maintain accurate patient records and prepare care plans
Perform nursing procedures as directed by the physician
Qualifications:
Registered Nurse with a valid state license
One year of clinical experience preferred
Home health experience and valid CPR certification are preferred
Must have reliable transportation and a valid driver’s license
