Job Description

We are seeking a Home Care RN to join our team, offering starting rates of $30 per hour and a $6,000 sign-on bonus. The role involves assessing and coordinating home care services for frail elders, ensuring holistic care and effective communication with the Interdisciplinary Team (IDT).

Key Responsibilities:

• Conduct assessments and develop care plans for participants.
• Coordinate home care services and ensure proper documentation.
• Provide skilled nursing care and educate participants and families.
• Assist in training and evaluating nursing staff.

Qualifications:

• Current RN license in Pennsylvania; BSN preferred.
• Minimum one year of experience with the elderly.
• Proficient in Microsoft Word and Excel.
