Home Care Registered Nurse – PRN

Job Description

Purpose: Provide health promotion, prevention activities, and home health nursing services across various settings.

Responsibilities:

• Assess populations and gather data for health assessments.
• Plan and provide services including physical assessments, education, and case management.
• Maintain accurate documentation and contribute to statistical data collection.
• Develop comprehensive care plans with measurable client goals.
• Participate in community planning and outreach activities.
• Support personal growth and assist in training new staff.

Qualifications:

• Valid Iowa RN license and accredited nursing degree.
• One year of clinical experience required.
• Strong communication, critical thinking, and nursing assessment skills.
• Ability to work collaboratively and provide services in diverse settings.

Working Conditions: Medium physical demands, flexibility in scheduling, and travel between locations required.
