Job Description

Position Summary: The Qualified Mental Health Professional – Community Services (QMHP-CS) provides Targeted Case Management and Rehabilitative Services to youth with serious emotional disturbances and their families, primarily in home or community settings. Responsibilities include delivering case management, documenting services, attending team meetings, and ensuring compliance with relevant regulations.

Qualifications:

• Bachelor’s or Master’s degree in a human service field.
• Valid state licensure (LCSW, LPC, LMFT) preferred.
• Knowledge of evidence-based treatment and strong assessment skills.
• Excellent communication and teamwork abilities.
• Experience in home/community-based services.
• CPR/First Aid and Behavior Management certifications.
