School-Based Case Manager – Youth Substance Abuse Services

Job Description

We are seeking a dedicated individual for a full or part-time position providing intensive case management to at-risk youth in community schools.

• Conduct case management meetings with families and stakeholders.
• Perform intake interviews, needs assessments, and service planning.
• Maintain documentation and conduct family meetings for effective communication.
• Meet with youth weekly, teaching self-regulation and executive functioning skills.
• Provide advocacy, referrals, and crisis support 24/7.
• Collaborate with clinicians and attend community meetings and trainings.
• Qualifications: BA in a related field and experience with adolescents.
