The Nurse Care Manager plays a vital role in supporting individuals facing mental illness, trauma, or addiction by developing and managing personalized care plans. This position involves advocating for consumers, linking them to necessary behavioral health and wellness services, and addressing non-clinical issues affecting their quality of life. The Nurse Care Manager will work under supervision, providing case management and wellness services in various settings, while collaborating with an integrated care team.
Qualifications:
- Associate’s degree in nursing and a valid Oklahoma RN license.
- Knowledge of integrated care and experience with serious mental illness.
- Strong communication skills and ability to work with diverse populations.
Responsibilities:
- Develop and implement individualized care plans.
- Monitor consumer needs and facilitate access to community resources.
- Maintain accurate documentation and participate in team meetings.
