RN Case Manager

Job Description

We are seeking a full-time Hospice Case Manager in Milwaukee, WI, with a Monday to Friday schedule (8am – 5pm) and occasional on-call duties. Candidates must have at least one year of nursing experience, a current RN license in Wisconsin, and CPR certification.

Compensation ranges from $88,000 to $92,000 plus mileage, with a comprehensive benefits package including medical plans, paid time off, wellness rewards, mental health support, and a 401(k) with company match.

Responsibilities include patient assessments, care plan development, documentation, and patient education. A valid driver’s license and reliable transportation are required.
