Job Summary:

This work-from-home position requires travel within the Charleston, SC area. The On-site Care Coordinator, a Registered Nurse, provides care coordination and education to Medicaid members in the Medically Complex Children’s Waiver program. Key responsibilities include:

• Conducting comprehensive assessments and scheduling home visits.
• Reviewing medical records and developing care plans.
• Educating parents/guardians and coordinating services.
• Maintaining communication with families and providers.

Qualifications:

• Minimum of three years of pediatric nursing experience.
• Bachelor’s degree in a health-related field preferred.
• Current RN licensure and BLS certification required.

Strong organizational and communication skills are essential.
