MDS Assessment Coordinator (RN or LPN)

Job Description

We are seeking a compassionate MDS Assessment Coordinator (LPN or RN) for an 80-bed skilled nursing facility in Plymouth, IN. This full-time position offers a salary of $74,000 – $88,000 annually, with a schedule from 8 AM to 5 PM.

Responsibilities:

• Conduct resident assessments and ensure compliance with regulations.
• Document health conditions and communicate findings.
• Coordinate timely completion of assessments with interdisciplinary teams.
• Develop individualized care plans based on Resident Assessment Protocols.
• Transmit MDS data to the state and maintain records.

Qualifications:

• Current RN or LPN licensure in Indiana.
• Minimum one year of experience in Long-Term Care as an MDS Coordinator.
• Must be 18 years or older.
