Job Description

Job Summary: The Nurse (RN or LPN) provides direct nursing and personal care to residents, supervises nursing assistants, and supports daily living activities to ensure residents’ health and safety.

Essential Duties:

• Monitor residents’ conditions and assess their needs.
• Prepare and administer medications as prescribed.
• Perform routine charting in accordance with policies.
• Manage admissions, transfers, and discharges of residents.

Qualifications:

• Nursing degree from an accredited program.
• Current, unencumbered RN or LPN license in Illinois.
• Eligibility to work in LTC per IDPH & HCWR guidelines.
