Registered Nurse Case Manager (Spanish Speaker)

Job Description

We are seeking a part-time Registered Nurse Case Manager with Spanish proficiency to join our interdisciplinary care team in Alameda County. The role focuses on high-touch care coordination for patients with chronic health conditions, particularly chronic kidney disease (CKD). Responsibilities include managing a caseload, conducting assessments, providing education, and collaborating with healthcare providers to improve patient outcomes and prevent hospitalizations.

Qualifications:

• Degree in Nursing and active California RN license
• Minimum of 2 years RN experience, preferably with chronically ill patients
• Bilingual in Spanish
• Willingness to travel up to 80% within Alameda County

Compensation: $67.56 per hour, with benefits including dental, vision, and retirement plans.
