Job Overview:

• Position: Case Manager IV
• Type: Part Time (0.65 FTE), Exempt, Days
• Sign-on bonus and relocation available for qualified candidates.

Role Summary:

• Provides clinically-based case management to enhance patient care.
• Integrates utilization management, care coordination, and transition planning.
• Accountable for a designated case load, focusing on patient needs and resource utilization.

Qualifications:

• Registered Nurse with MSN (5 years experience) or BSN (10 years experience) in case management.
• Current NM RN licensure and BLS certification required.
• Two years clinical nursing experience preferred; national certification is a plus.

Compensation: Up to $51.41/hr, based on experience and qualifications.
