RN Care Manager – Hybrid (Bengali/Arabic/Chinese Languages Preferred)

Job Description

We are seeking an RN Care Manager to join our multidisciplinary team, focusing on improving the quality of life for members in need of additional care and support at home. The role involves:

• Conducting telephonic assessments and care planning.
• Coordinating healthcare services and member education.
• Initiating utilization reviews and collaborating with healthcare providers.
• Participating in quality improvement initiatives and case rounds.

Requirements:

• Active RN license and strong communication skills.

Preferred:

• Bachelor’s in Nursing, CCM certification, and experience in managed care.
