Assistant to the Director of Clinical Services (DCSA) – LPN Required

Job Description

Job Summary: The Director of Clinical Services Assistant (DCSA) supports field clinicians and patients under the supervision of the Director of Clinical Services, focusing on community resources to meet individual and family needs.

Schedule: Friday to Sunday, 8:00 AM to 8:00 PM (Full-time) or 8:00 AM to 5:00 PM (Part-time).

Essential Responsibilities:

• Evaluate patient charts and monitor care plans.
• Schedule CNA visits and report changes in patient conditions.
• Coordinate communication among interdisciplinary team members.
• Supervise ancillary personnel and participate in quality improvement activities.

Qualifications:

• Graduate of an accredited nursing school with 1-2 years of acute care and home care experience.
• Current nursing licensure and CPR certification required.
• Excellent communication and nursing skills.
