We are seeking a Licensed Speech-Language Pathologist (SLP) for a new care coordination program aimed at enhancing the lives of seniors in various living settings. This role allows you to provide speech-language pathology services while engaging in care coordination tasks, offering flexible scheduling and career growth opportunities.

Responsibilities include:

  • Delivering speech-language pathology and wellness services to older adults.
  • Conducting assessments and therapeutic interventions as part of a multidisciplinary team.
  • Supporting care coordination through wellness visits and communication with providers.
  • Maintaining ethical standards and contributing to program development.

    • Qualifications:

  • Completion of an accredited SLP program (New Grads welcome).
  • Active ASHA certification and state licensure.
  • Strong communication skills and a results-oriented mindset.
