Nurse Practitioner for In-home Visits

Job Description

We are seeking board-certified nurse practitioner contractors to conduct in-home health evaluations for Medicare Advantage, Medicaid, and select commercial plan members. Responsibilities include:

• Conducting in-home visits focused on overall health and identifying barriers to care.
• Compensation of $2,500 to $4,000 weekly, with a base rate of $80 per assessment.
• Potential to increase per-visit rates by up to 135% in specific counties.
• Flexible scheduling available, with shifts from Monday to Sunday (no nights/weekends).
• Independent contractor status with malpractice coverage and travel reimbursements available.
