We are seeking an experienced Home Health RN Case Manager for a full-time position in the Wakefield/Stoneham/Winchester/Malden/Melrose, MA area. This role offers a Monday-Friday schedule with no weekends, on-call, or holiday shifts required. Candidates must have at least 1 year of recent Home Care RN Case Management experience, including OASIS visits with a Medicare certified agency.
