Immanuel Pathways PACE® is seeking a Primary Nurse Case Manager for its Central Iowa location. This full-time position offers a Monday to Friday schedule from 8:00 am to 4:30 pm, with pay based on experience and qualifications. Responsibilities include assessing participants’ health needs, coordinating care with the Interdisciplinary Team, and providing skilled nursing care.
