Job Description

We are seeking RN Case Managers passionate about hospice care to join our unified team. This full-time position involves overseeing patient care plans, collaborating with an interdisciplinary team, and ensuring quality care delivery. Candidates must hold a current RN license in Pennsylvania, CPR certification, and have at least one year of hospice experience. We offer competitive pay ($65,000 – $85,000), a sign-on bonus (up to $15,000), flexible schedules, and comprehensive benefits including 401(k) and health insurance. Relocation to Wexford, PA is required.
