> REF:#f30d60760c8a3fef *Position Summary*Plans and provides physical services to services clients in their homes in accordance with the Plan of Care and Agency policies and procedures. The Occupational Therapist performs functions that require substantial specialized knowledge, judgment and skill and must be able to make judgments accordingly.
*Qualifications*
* Graduated after successful completion of an Occupational Therapist program accredited by the Accreditation Council for Occupational Therapy Education (ACOTE) of the American Occupational Therapy Association, Inc. (AOTA), or successor organizations of ACOTE: and is eligible to take, or has successfully completed the entry-level certification examination for Occupational Therapists developed and administered by the National Board for Certification in Occupational Therapy, Inc. (NBCOT)
* Is licensed or otherwise regulated, if applicable, as an Occupational Therapist by the state in which practicing, unless licensure does not apply.
* Minimum of two (2) year experience in a health care facility or equivalent experience preferred.
* Demonstrated good written and verbal communication skills interpersonal skills.
* Licensed driver with automobile insurance in accordance with state/Agency requirements. Good driving record and reliable transportation.
* Current CPR certification.
*ESSENTIAL FUNCTIONS/AREAS OF ACCOUNTABILITY*
* Performs occupational therapy assessments, diagnostic tests, skilled procedures, and ongoing evaluation of clients who are receiving services under a medically approved Plan of Care.
* Completes assessments in a timely manner and in accordance with Agency policy.
* Develops a therapy care plan to provide direction to other.
* Provides assessment findings and goals to assist in developing the client care plan and obtains specific physician orders as needed.
* Documents all findings, plans, interventions, and client progress (outcomes) on designed forms and submit per Agency guidelines, every (48) hours.
* Communicates plans and changes to the physician and to Nurse Manager and other Agency staff through the care plan, written progress notes, and participation in case conferences.
* Guides and instructs the client in prescribing therapeutic self-care and creative activities that are directed toward improving independence and physical and cognitive functioning.
* Advises, consults, and teaches appropriate treatment procedures and supportive activities to clients, family members, and other Agency staff.
* Designs and/or orders appropriate adaptive/assistive devices.
* Instructs clients and their families in the client specific program.
* Performs job in compliance with Agency policies and procedures and professional and community standards.
* Provides services in accordance with applicable state, federal, and ACHC standards.
* Attends meetings and required educational programs.
* Accepts responsibility for personal and professional development and identities learning/developmental needs to supervisor.
* Attends care conferences, initiates interdisciplinary referrals and participates in care coordination as needed.
* Serves on the clinical Record Review Committee.
* Performs their duties and responsibilities as deemed necessary.
*SUPERVISION OF OCCUPATIONAL THERAPY ASSISTANTS*
* Instructs and supervises Occupational Therapy Assistants in certain phases of occupational therapy.
* Available by telephone to the Occupational Therapy Assistant at all times when an Occupational Therapy Assistant is providing treatment to a client.
* The Therapist performs an on-site supervisory visit with or without the COTA present every 30 days.
* Completes an on-site functional assessment, review, and revision, of the Plan of Care, and an assessment of utilization of community resources.
