We are offering a $6,000 Sign-on Bonus (to be paid throughout the first year of employment).

The Advanced Care Center of Butler is a comfortable and home-like 29-bed skilled nursing center that provides high quality short-term rehabilitation services and comprehensive nursing care to help individuals successfully return to home.

In cooperation with the DON, assumes responsibility of total resident/patient care on a designated unit for each shift worked by performing professional nursing duties and assuring the completion of nurse aide duties. Cooperates with all disciplines to assure high quality resident/patient care.

Maintain charge and direction of unit in cooperation with the unit coordinator to assure quality resident/patient care is provided.
Create nurse aide assignments if needed to meet the needs of the resident/patients.
Notify supervisor (DON) when resident/patient condition deteriorates or there is a significant change in condition or resident/patient becomes unstable.
Administer medications and treatments in accordance with established policies and procedures.
Attend and participate in care planning conferences as directed and/or in absence of DON.
Update resident/patient care plans on a daily basis as needed.
Assist in completion of MDS’s quarterly reviews, monthly summaries and other documentation related to resident/patient care daily or as necessary/scheduled.
Appropriately assess residents/patients and follow through according to established policies and procedures.
Assist with resident/patient ADL’s as necessary.
Assist with resident/patient meals and fluid intake on a daily basis to assure proper nutrition is maintained.
Accompany DON and/or physician on resident/patient visits as needed to assure continuity of care.
Transcribe physician orders per policy in a timely manner.
Observe nursing care and visit residents/patients throughout the shift to assure that nursing care is carried out as directed. Report incomplete tasks to the DON.
Participate in receiving report from off-going shift and giving report on-coming shift to assure continuity of care. Communicate necessary information to nurse aides.
Monitor and oversee care provided by Nurse Aides.
Complete resident/patient incident/accident reports per policy in a timely manner.
Collects samples, such as urine or sputum, from residents/patients for testing, as needed.
Communicates information as necessary with families, residents/patients, staff, etc.
Assure that appropriate and adequate supplies are available on unit for all disciplines.
Reports any nursing assistant performance or care issues to the unit DON.
Participate in unit team meetings to provide and enhance communication between all disciplines on all shifts.
Participate in problem resolution regarding employees and other unit issues.
Participate in orientation and training of personnel.
Assure a clean, safe and orderly environment for residents/patients, staff and visitors.
Accurately document all care provided to resident/patient during shift and before the end of the shift.
Chart medications immediately after administered, per policy.
Provide input to unit coordinators concerning nurse aide performance.
Maintain confidentiality.
Attend and participate in internal and external meetings, in-services, mandatory trainings/in-services and continuing education programs.
Provides IV services within the PA Functions of a Registered Nurse.
Always functions within the limits of the Pennsylvania State Professional Standards Title 49 Chapter and in accordance with the scope of the practice permitted with the licensure.
Performs other duties as assigned.

Graduated from an accredited school of Nursing and must possess a current Registered Nurse license, in good standing, from the Commonwealth of Pennsylvania.
