Advanced Home Health and Hospice of Northern Idaho is looking for a Full Time or Part Time Physical Therapist (PT) to join our growing home health! This position will see patients around the Lewiston & Moscow area. Candidates will be friendly, positive and focused on excellence! Advanced Home Health is an agency where the focus is on delivering 5-star services and exceptional patient outcomes.

*Job Responsibilities:*
* Perform admission, discharge, and routine home health visits.
* Coordinate & communicate patient care plans with interdisciplinary team.
* Document all phone calls and visits using our EMR.

Job Types: Full-time, Part-time

Pay: $75,000.00 – $95,000.00 per year

Expected hours: 15 – 40 per week
