Heritage Center and Magnolia Gardens, located in Stoughton, WI, is hiring for a Health and Wellness Nurse. This full-time, benefit eligible position will be primarily located at our CBRF on the Skaalen campus.

The Health and Wellness Nurse is responsible for monitoring personal care services and medication administration provided to residents by trained staff, promoting the health and well being of each resident, and reporting observed changes in tenant condition or health to the physician, Director and/or tenant’s responsible party.

A successful Candidate will be:
* A Licensed Registered Nurse (RN/LPN) in the state of Wisconsin and in good standing.
* Experienced in Long Term Care Experience (Preferred)
* Self-motivated and independent
* Confident to make decisions autonomously
* Able to lead a team of care workers to provide consistent, quality care to tenants
