Location: Hempstead, NY (Facility-based)

Salary: $150,000 – $160,000 per year

Schedule: Monday to Thursday, 9:00 AM – 5:00 PM, Friday 9:00 AM – 3:00 PM

About the Facility: Our client, a well-established healthcare provider in Hempstead, is looking for a Spanish-speaking Nurse Practitioner (NP) to join their team for virtual establishment of care visits for home care patients. This role will involve working in the Hempstead facility while conducting virtual visits to establish care and provide follow-up care to patients.

Position Overview: As an NP, you will conduct virtual visits for home care patients, assessing their medical needs, establishing care plans, and ensuring the continuation of proper treatment. Fluency in Spanish is required for effective communication with the clinic’s patient base.

Key Responsibilities:

Perform virtual visits to establish care and evaluate the health of home care patients.
Provide follow-up care, ensuring patients are adhering to their prescribed treatment plans.
Collaborate with the healthcare team to ensure continuity of care and address patient concerns.
Educate patients about their health conditions and treatment plans.
Maintain accurate and up-to-date patient records.

Qualifications:

Licensed Nurse Practitioner (NP) in the state of New York.
Fluency in Spanish is required.
Experience in home care or virtual healthcare settings is preferred.
Strong clinical, communication, and organizational skills.
