> REF:#44902279e5cb1937 An LMHP will provide specialized services that include comprehensive screening and assessments to better understand the youth and family dynamics using evidence-based programming, proven to improve family functioning by increasing family problem-solving skills, enhancing emotional connections, and strengthening social networks.*Duties & Responsibilities*
1. Complete assessment and intake services promptly for all assigned clients.
2. Develop individualized service plans in conjunction with the family and other agencies/professionals.
3. Document all services in accordance with Community Works policy and procedures and submit them in a timely fashion. Ensure Medicaid and insurance guidelines compliance, as applicable.
4. Plan and administer therapeutic treatment identified in the individualized service plan. Change method and degree of therapy when indicated by client reaction/needs.
5. Collaborate with other professionals, family, and agency clinical staff to ensure consistency and timely completion of goals identified in the ISP.
6. Maintain regular contact with other agencies/professionals and family regarding client progress via phone calls, letters, emails, and/or monthly reporting. Prepare court reports as needed.
7. Attends agency-required meetings, i.e., staff meetings, treatment team meetings, and IEP meetings, as required.
8. Comply with certification/licensing standards and provide all related documentation to the administrative office.
*Minimum Requirements*
*Education:*
· Master’s degree in human services field.
· Current license with the State of Oklahoma / or candidacy
*Experience:*
· At least two years of experience in the human services field
*Physical:*
· Must be able to lift & carry up to 25 pounds.
· Full range of body motions
· Ability to spend 80%-90% of time standing, walking, kneeling, and/or reaching to perform office duties.
· Sight, hearing, touch, & speaking is required 100% of the time to perform office duties & interact with clients, consumers
